# Penetration (laryngeal penetration): plain-language definition

Penetration means food or liquid entered the voice box but stayed above the vocal folds, without passing below them into the windpipe.

## In plain language

The larynx, or voice box, sits at the top of the windpipe, and the vocal folds are inside it. The space just above the vocal folds is called the laryngeal vestibule. It helps to picture it as a small entryway leading to the door.

Penetration means some food or liquid entered that entryway but stayed above the vocal folds. It did not pass below them. That location is exactly what separates the two words families see most often in a report: penetration is above the vocal folds, and aspiration is below them. A report may say laryngeal penetration to be specific about where.

Reports often add detail about how far into the entryway the material went, and whether it was cleared back out — for example by coughing or throat clearing — or whether it stayed. Clinicians may record these observations using the Penetration-Aspiration Scale, an 8-point scale used to describe penetration and aspiration events seen on the study.


## Why a clinician looks at this

Clinicians track penetration separately from aspiration because the two words describe different places. Noting where material went, how far in, and whether the person cleared it back out gives the team a precise picture of how the airway responded during that swallow, rather than a single yes-or-no answer.
