What is the difference between starch and xanthan gum thickeners?
Starch thickeners are broken down by amylase, the enzyme in saliva, so a drink can thin out inside the mouth within seconds. Xanthan gum resists amylase and holds its thickness. Both are used clinically. Which one is right is a decision for your SLP or dietitian, and either way you confirm the batch with the flow test.
In a 2015 study in Annals of Rehabilitation Medicine, volunteers held thickened water in the mouth for 20 seconds before it was measured. The starch-based product fell from about 1,300 mPa·s to 187 mPa·s, losing roughly 85% of its thickness. The gum-containing product held about 616 mPa·s, near half its starting value. The authors concluded that the gums protect the starch from being broken down by salivary amylase.
That gap matters because the mouth is where a drink waits before the swallow. A starch-thickened drink that measured at the prescribed level in the cup may no longer be at that level by the time it reaches the throat, particularly for someone who holds liquid in the mouth for a while before swallowing.
Timing cuts the other way too. Starch thickeners keep thickening as they stand, so a drink mixed correctly can be too thick twenty minutes later. Xanthan-based products are more stable over time and temperature, which is part of why they are often chosen for hot drinks.
None of this makes one type universally better for a given person. Taste, cost, availability, how the product behaves with the drinks someone actually likes, and the clinical picture all factor in. Ask which type is prescribed, and tell the care team before switching brands or types rather than after.
