Cardizem CD, Cardizem LA, and other extended-release diltiazem products are on the ISMP “Do Not Crush” list; crushing releases a day's worth of drug at once instead of gradually.

Ask the pharmacist whether an immediate-release diltiazem tablet regimen or an ER capsule brand whose label allows opening and sprinkling the beads is an option.
