Why
Glucophage XR, Fortamet, and Glumetza are all on the ISMP “Do Not Crush” list as extended-release tablets, and FDA labeling states Glucophage XR must be swallowed whole and never crushed or chewed. Crushing destroys the slow-release mechanism.
What to ask the pharmacist instead
Ask the pharmacist whether switching to immediate-release metformin tablets or a metformin oral solution is an option.
